Mild/Moderate Special Education Distance Education Program

Program Advisor

Melanie JonesMelanie Jones is the program advisor for the Mild/Moderate Special Education Distance Education Program. Melanie can be reached by email or by phone at melaniej@cc.usu.edu  or 435-797-7575 (phone messages will be checked on a regular basis). Melanie assists students in planning their courses as they prepare to apply to the program. She also provides information and assistance to students who are enrolled in this distance education program throughout their studies.

Admission

Admission of students into the Mild/Moderate Special Education Distance Education Program occurs on October 1 the year prior to the beginning of the program. Applicants who do not meet the minimum admission requirements will not be considered. The following documents contain information regarding admission, the requirements for admission, and a basic checklist for students who are working to meet these minimum requirements.

Applications for admission to this program are available below as both HTML and PDF documents. You may print an application, complete it, and mail it to the address on the application. Applications are due before October 1 the year prior to the beginning of the program (applications for the 2007 program are due before October 1, 2006).
